American Form Corporation has been in the concrete business since 1978 — 48 years, according to its Better Business Bureau profile — run by president Leonard Mills from Winthrop Street in Rehoboth, with a second address across the line in East Providence, Rhode Island. BBB grades the company A+ and lists it under concrete contractors, though it is not BBB accredited. It is a small outfit: BBB records a single employee. What you won't find is any account of the work itself. Neither BBB nor BuildZoom carries a customer review, and BuildZoom shows no permit history for the company at all — its score of 90 there, as BuildZoom itself explains, indicates only that the business is licensed or registered somewhere. The registrations on file are Rhode Island ones, held with that state's Contractors' Registration and Licensing Board: a Residential/Commercial licence #GC-2087 that BuildZoom says it checked as active in July 2026, and a Residential Contractor licence #2087 whose expiration BuildZoom last saw back in July 2022. Both are the directory's records rather than a state confirmation. A long-established name for concrete and foundation work on the Rehoboth–East Providence line. Ask for a few recent job addresses you can look at, and confirm which registration covers work on the Massachusetts side before signing.
